Dark Souls II wiki user Lotus Gramarye has uploaded a list of banned names from Dark Souls II, and it certainly makes for interesting reading.
The list has over 1200 entries - most of them are really, really childish, but there also some odd inclusions, such as Zorch (ostensibly a band from the 1970s).
You may want to have your digital copy of Urban Dictionary on hand when browsing through the list - there are some pretty obscure entries on the list.
