The item is how it sounds a lump of coal, when you pick it up Isaac's face will be smudged with coal causing him to have black spots on his face. The lump causes the shots to turn grey which do more damage the farther they have travled by becoming bigger. This item is unlocked by beating Krampus in Devil Rooms (randomly appears in them) and is dropped by Krampus (100% of the Time).
