Pills are consumables found throughout the game that have a multitude of effects. Every run the effect of each colored pill is changed making it impossible without using the pill or having PHD to know what effect it has. Pills come in three general forms stat upgrade, stat downgrade or neutral . The first two are straight foward with the pill either raising or lowering a certain stat, the final one affects your character in a few ways liike getting hurt or having bad gas. While upgrade pills will raise the stat by 1, the downgrade pill will only lower it by .8 (except for health down which lowers it by a heart container). The only way to avoid getting downgrade pills is either to get PHD which changes all downgrade pills into their upgrade form and lets you see what pills it is before using it, or having Lucky Foot which raises your luck and makes all downgrade pills into upgrade pills.
Pill List
Bad Gas: Gives you gas which will poison any enemies near by.
Balls of Steel: Gives you two soul hearts.
Bombs are Keys: Switches the amount of bombs and keys (another can be used to change it back).
I Found Pills: Makes you have give off a durr sound and changes how you look. In Wrath of the Lamb this pill can also give you puberty effect which lets you grow a few hairs on your head.
Pretty Fly: Spawns a pretty fly to fly around you (If you already have three objects around you nothing happens.)
Bad Trip: Does one heart container of damage, this pill will turn into full health if you are down to one red heart. But if this is used one ???/Blue Baby with one soul heart left it will kill him.
Full Health: Will restore all empty heart containers, when playing as ???/Blue Baby it will restore up to 3 Soul Hearts total.
Telepills: When used this pill will teleport your to a random room on the map, there is a 5% chance when used will transport you to a I AM ERROR room.
Health Up: Gives you a new empty heart container and tints you red.
Health Down: Removes a heart container, if you only have one heart container then it turns into a Health Up.
Speed Up: Raises your base speed and tints you yellow.
Speed Down: Lowers your base speed.
Tears Up: Raises your firing rate of tears and tints you purple.
Tears Down: Lowers your firing rate of tears.
Range Up: Raises your range and tints you blue.
Range Down: Lowers your range.

Puberty: Gives you pubic hair on your head.
Friends Till !he End!: Spawns three homing flies.
Luck Up: Raises your luck score and tints you green.
Luck Down: Lowers your luck score.
