When on the ground it is a black ankh (An Egyption hieroglyphic), after Isaac picks it up he wears it around his neck and gives his eyes shadows. This item activates after you die, upon death you come back to life in the previous room as ???/Blue Baby, all your heart containers turn into spirit hearts (If you playing as ??? you revive with just 3 spirt hearts). If you beat a boss like Isaac after coming back to life you will unlock the achivment for killing Isaac with ??? rather then the orignal character you played as. Ankh is found in Secret Rooms.
