The Rat Plague is an epidemic that has devastated the city of Dunwall, that is spread through rats. It has killed about half of the population of Dunwall, with the first victim being a young boy. This young boy was bestowed powers by The Outsider, with one of the rats summoned by the boy biting and infecting him.
Those infected with the Rat Plague have symptoms that include discolored skin, bleeding from the eyes, weight loss and thinning of the hair. The government in power use Walls of Light to quarantine those infected, who they call "weepers," as well as implementing a curfew.
